Abstract

GASTROINTESTINAL STROMAL TUMOR OF THE STOMACH AS AN ATYPICAL CAUSE OF BLE E DING FROM THE UPPER DIGESTIVE TRACT – A CASE REPORT

Introduction. Mesenchymal tumors in the digestive tract belong to the group of rare neoplasms coming from multipotential mesenchymal stem cells. Course of the illness may be characterized by non-specific symptoms: nausea, vomiting, constipation, melena, stomachache or anemia.

Aim. The aim of this study was to present an atypical cause of bleeding from the upper digestive tract.

Case study. In May 2012, a 72-year-old man, with stromal tumor of the stomach diagnosed in July 2011, was readmitted to hospital because of increasing weakness, intensifying difficulty in breathing, epigastric pain, persistent vomiting, loosing appetite and melena.
Medical examination revealed paleness and pale mucous membranes, irregular heartbeat (100/min), blood pressure 100/80 mm Hg. His abdomen was soft, without pathological resistance, tender in the epigastrium. Peritoneal signs were negative. Per rectal examination revealed melena in the rectum.
During the gastroscopy, an exophytic lesion with diameter of 2 cm contact bleeding in the upper part of the gastric corpus in the greater curvature on the posterior wall was found. The abdominal ultrasonography did not show any gastric abnormalities.

Conclusion. Gastrointestinal stromal tumors represent a rare cause of bleeding from the upper digestive tract, whereas the bleeding can be the first clinical presentation of this kind of tumors.
