Abstrakt

ANALIZA RETROSPEKTYWNA LICZBY PORODÓW I JEJ WPŁYWU NA WYNIKI POŁOŻNICZE U MATEK ORAZ WYNIKI NOWORODKÓW W PRZYPADKU PORODÓW O CZASIE

Cel pracy. Tematyka związku pomiędzy rodnością a wynikami ciążowymi w dalszym ciągu pozostaje aktualna, ponieważ przeszłość porodowa kobiety przekłada się na ocenę ryzyka kolejnych ciąż oraz potencjalnych zagrożeń dla zdrowia i życia kobiety, jak i jej dziecka. Dlatego celem badań była analiza związku pomiędzy rodnością a wybranymi wynikami matczynymi i noworodkowymi związanymi z porodem w terminie.

Materiał i metody. To retrospektywne badanie przeprowadzono w okresie od stycznia 2017 do grudnia 2021 roku na podstawie dokumentacji medycznej ze Specjalistycznego Szpitala św. Zofii w Warszawie (Polska). Przeanalizowano 26 862 przypadków porodów o czasie.

Wyniki. U pierworódek rodzących o czasie zaobserwowano wyższy iloraz szans wystąpienia profilaktyki przeciwzakrzepowej (OR = 1,13), stanu przedrzucawkowego (OR = 3,12), nadciśnienia indukowanego ciążą (OR = 1,58), niedoczynności tarczycy (OR = 1,24), niedokrwistości (OR = 1,73) oraz wewnątrzmacicznego ograniczenia wzrastania płodu (OR = 1,93). Wieloródki istotnie częściej rodziły w domu narodzin, drogami natury, dochodziło u nich częściej do pęknięcia krocza (p < 0,05).

Wnioski. To badanie pokazało, że pierwiastki są szczególną grupą kobiet, które są narażone na więcej powikłań okołoporodowych i procedur podczas porodu w porównaniu do wieloródek. Należy objąć je szczególną opieką, ponieważ doświadczenia związane z pierwszym porodem mogą wpływać na dalsze decyzje prokreacyjne.
