Przedwczesny wytrysk (premature ejaculation, PE) obok zaburzeń erekcji (erectile dysfunction, ED) należy do najczęściej zgłaszanych przez mężczyzn dysfunkcji seksualnych. PE dotyczy mężczyzn z każdej z grup wiekowych i znacząco obniża jakość życia nie tylko mężczyzny, ale również jego partnerki. Pomimo dużego rozpowszechnienia w populacji ogólnej, zaburzenie to nie zostało do chwili obecnej dobrze poznane. Pacjenci niechętnie informują lekarzy o występujących u nich objawach, są przekonani o braku skutecznych środków leczenia, a lekarze rodzinni nie dysponują wystarczającą wiedzą na temat symptomów i dostępnych metod terapii. Oddziaływaniami terapeutycznymi powinien zostać objęty zarówno mężczyzna, jak i jego partnerka (dotyczy to głównie psychoterapii i psychoedukacji).
Głównym celem terapii jest wydłużenie czasu śródpochwowej latencji wytrysku (intravaginal ejculation latency time, IELT), uzyskanie kontroli nad ejakulacją oraz zwiększenie satysfakcji ze współżycia seksualnego u obojga partnerów. Do najbardziej efektywnych leków w terapii PE zalicza się trójpierścieniowe leki przeciwdepresyjne, inhibitory wychwytu zwrotnego serotoniny (selective serotonin reuptake inhibitors, SSRI), w tym dapoksetynę, leki z grupy SARI (serotonin antagonist and reuptake inhibitor) oraz inhibitory fosfodiesterazy typu 5 (phosphodiesterasetype 5 inhibitors, PDE-5), a obok nich środki znieczulające miejscowo. Celem części pierwszej pracy jest charakterystyka, ocena rozpowszechnienia oraz przedstawienie współczesnych klasyfikacji PE. W części drugiej, autorzy zamieścili podstawowe zagadnienia związane z psychoterapią oraz obecnie dostępnym leczeniem biologicznym przedwczesnego wytrysku.
