Trudności w regulacji emocji a częstość i ciężkość niesamobójczych samouszkodzeń u adolescentów hospitalizowanych psychiatrycznie: badanie kliniczne i korelacyjne

Wstęp: Niesamobójcze samouszkodzenia (NSSI) stanowią istotny problem zdrowia psychicznego w populacji adolescentów. Zachowania te definiowane są jako intencjonalne uszkodzenia ciała podejmowane bez zamiaru odebrania sobie życia i najczęściej pełnią funkcję regulacji napięcia emocjonalnego. W literaturze wskazuje się, że jednym z kluczowych czynników sprzyjających występowaniu NSSI są trudności w regulacji emocji.
Cel badania: Celem pracy było określenie związku między nasileniem niesamobójczych samouszkodzeń, rozumianym jako ich liczba i dotkliwość, a poziomem trudności w regulacji emocji u adolescentów hospitalizowanych psychiatrycznie.
Materiał i metody: Badanie miało charakter przekrojowy i zostało przeprowadzone w latach 2019–2022 wśród 205 adolescentów hospitalizowanych psychiatrycznie (wiek 12–24 lata). Do oceny trudności w regulacji emocji wykorzystano skalę Difficulties in Emotion Regulation Scale (DERS), natomiast nasilenie NSSI oceniano za pomocą Self-Injury Trauma Scale (SIT), obejmującej indeks liczby (NI) i indeks dotkliwości samouszkodzeń (SI). Analizy statystyczne przeprowadzono w programie IBM SPSS Statistics 28, wykorzystując statystyki opisowe, analizę korelacji Pearsona oraz analizę regresji wielorakiej.
Wyniki: Stwierdzono dodatnie, choć słabe korelacje między ogólnym poziomem trudności w regulacji emocji a liczbą (r = 0,256; p = 0,001) oraz dotkliwością samouszkodzeń (r = 0,204; p = 0,001). Najsilniejsze zależności dotyczyły trudności w kontroli impulsów oraz problemów z zachowaniem ukierunkowanym na cel. Analiza regresji wykazała, że trudność w kontroli impulsów była istotnym predyktorem zarówno liczby (β = 0,265; p = 0,009), jak i dotkliwości samouszkodzeń (β = 0,220; p = 0,015).
Wnioski: Uzyskane wyniki potwierdzają istotną rolę dysregulacji emocjonalnej w nasileniu zachowań NSSI u hospitalizowanych adolescentów. Szczególne znaczenie wydaje się mieć deficyt kontroli impulsów, który może zwiększać zarówno częstotliwość, jak i dotkliwość samouszkodzeń. Wyniki podkreślają potrzebę uwzględniania treningu regulacji emocji w programach terapeutycznych dla młodzieży z zachowaniami autoagresywnymi.
